Family Medical Leave
Understanding Family Medical Leave
Family Care and Medical Leave (FML) is available to eligible employees in accordance with the Federal Family and Medical Leave Act (FMLA), the California Family Rights Act (CFRA) and Collective Bargaining Unit Agreements. FML may be used for yourself or an eligible family member.
Upon completion of FML, you are entitled to return to the same or similar position with equivalent benefits, pay and conditions of employment. You are also entitled to retain any health insurance benefits during time away from work due to FML.

Process to Request to Use Family Medical Leave

Complete the Certification of Qualifying Exigency for Military Family Leave (FML) [pdf] and forward to the Leave Program Manager along with the Certification of Serious Injury or Illness of Covered Service Member Military Family Leave (FML) [pdf].
- For yourself or an eligible family member:
- Complete the Leave of Absence Request Form [pdf] and forward to the Leave Program Manager along with the Medical Certification for Family Medical Leave [pdf].
- For a qualifying exigency for military family leave:
Process to Return to Work
Complete the Return to Work Certification [pdf] and forward to the Leave Program Manager. You may not return to work without the approval of the Leave Program Manager.
